forward.

THE BOTTOM OF THIS PAGE. FILL OUT THE ONLINE FORM AND HIT THE SUBMIT
BUTTON ONCE. BE CAREFUL THAT ALL INFORMATION GIVEN IS ACCURATE.
UNAUTHORIZED LISTING OF OUR COMPANY NAME AS U.S. AGENT IS STRICTLY PROHIBITED.
STEP 2. WIRE YOUR PAYMENT ACCORDING TO THE FEE SCHEDULE BELOW. IF PAYING BY CHECK OR DRAFT PLEASE BE SURE IT IS A U.S. DOLLAR CHECK DRAWN ON A U.S. BANK.
ACTUAL REGISTRATION PROCESS WILL NOT BEGIN UNTIL FUNDS ARE RECEIVED.
STEP 3. WE WILL SEND YOU A CONFIRMATION UPON RECEIPT OF YOUR FORM. ONCE THE REGISTRATION PROCESS IS COMPLETE WE WILL INFORM YOU OF YOUR FDA REGISTRATION NUMBER.
Please note that in order to provide separate registration
numbers (for each exporting company) it is necessary to fill out and submit
separate forms. Also, the registration process is due to be completed by
December 12, 2003. Shipments thereafter by non-registered companies are
subject to being held within the port of entry.
If at any time, an element of the required information given by your company
changes, the FDA requires that this information must be given to us within 60
days of the change in order to amend your registration.
Furthermore, if a company goes out of business, notification must be given that
the registration is cancelled.
